Join Andwell Health Partners staff as they share the exciting launch of a dementia care management program (coming July 2025) and the importance of advance care planning. While advance care planning is crucial for patients with dementia and their caregivers it is something everyone should consider; it’s a gift to yourself and those you love when you no longer can vocalize your wishes. Join us to learn more on National Healthcare Decision Making Day, April 16, 2025. 

Meet our Presenters: 

Sherri Tanner is a registered nurse who has dedicated over 30 years to growing in her profession at Andwell Health Partners. Sherri started her career as a nurse’s aide, then a visiting nurse, up to program manager. Most recently, Sherri became a Certified Dementia Practitioner—a testimonial to her passion for supporting Maine’s aging population and their caregivers, helping to keep them safe at home. 

Christine (Chris) Foss has worked in and with the long-term care industry since 2005. Chris is passionate about cultivating meaningful relationships with residents, their care partners, and the people who serve them. Chris is passionate about providing valuable resources for care partners and enhancing patients’ and family’s experiences throughout the continuum of care. 

Michelle Tilton is a licensed social worker with 25 years of experience caring for patients in hospice, palliative care, home health, substance use, inpatient behavioral health, and child development. Michelle is passionate about advocacy for patients’ rights, health literacy, and education. Together, Michelle and her team help patients navigate their illness, providing compassionate care, advocacy, and collaboration with a focus on what matters to the patient. 

Reverend Sarah Gillespie is a board-certified chaplain, working in healthcare since 2015. Sarah leads a team of hospice chaplains who offer multi-faith spiritual care at end-of-life. Sarah is passionate about educating on advanced care planning, spiritual pain and distress, and navigating grief and burnout for healthcare workers.
